Students: do you need working papers?

Do you need WORKING PAPERS?

Go to the main office (240) and pick up an application form.

-Complete the application.
-Have your parent/guardian sign the application.
-Attach copies of your BIRTH CERTIFICATE, SOCIAL SECURITY CARD and a letter of CERTIFICATION of PHYSICAL FITNESS SIGNED BY YOUR DOCTOR

Bring all of the above to Ms. Gladden in the main office, room 240.
